Default Need quick help with PHP and Dreamweaver

I'm trying to use PHP in Dreamweaver to connect and insert new records in my SQLDatabase. I have gotten Dreamweaver to connect to the database, but I'm not sure how to update the database using Dreamweaver forms. Does anyone know how to do this?
    I have this script,
"//Insert a row of information into a table
    mysql_query("INSERT INTO table (column1,column2) VALUES ('value1','value2')");"

I know it will insert the values that I type in, but is there anyway that I can make it insert information that is put into a Dreamweaver form?

Thanks in advance for your help!

    Default Record Insert Page

I always create a special insert page that is bound to the database. You basically need to bind the form fields to the database fields. Adobe has a good tutorial that explains how to do this easily.

Default Sorry, but I still need major help.

Thanks for the url. It will be great once I get the connection to work.
So when I just use PHP, I can connect to the SQL database, but when I use dreamweaver, I can't.
I go to Bindings, and then hook it up to the site. Something keeps going wrong, and I'm not sure what I am doing. Can you help? Here is what I have entered.

    LOCAL INFO

    SITE NAME: my web address
LOCAL ROOT FOLDER: the folder that all my files are in on MY computer
    ADDRESS: http://www.myaddress.com/

    REMOTE INFO

    ACCESS: FTP
    FTP HOST: my web address
    HOST DIRECTORY: /var/www/html
    LOGIN: webaddress@webaddress.com
    PASSWORD: my password
(It says it connects to the server correctly)

    TESTING SERVER

    SERVER MODEL: PHP MySQL
    ACCESS: FTP
FTP HOST: the folder that my files are in on MY computer (I think this one is the problem???)
    HOST DIRECTORY: /var/www/html
    LOGIN: same as before
    PASSWORD: same as before

*ONCE I GET THAT RIGHT,I ALSO HAVE ANOTHER QUESTION
When I go to connect to the SQL database, I have to enter the "MySQL server", and I'm not sure what to put. Do you know?

See my reply to your other post. The problem I think lies in the URL prefix setting under Testing Server.
